S123 Website's "Report name" should include the template name

Minor thing, but the "Report name" field should use the selected template rather than the Feature Layer.

The current format is: <Feature Layer name>_OID<Object ID><timestamp>

Where you can upload multiple Feature Reports, you are also testing those Feature Reports. Getting output files with nearly identical names is annoying. Especially if you're like me and do multiple tests back-to-back:
