'Analyze Survey' feature incorrectly identifies 'form_title' when 'generated_note_form_title' is set

When using 'Analyze Survey' while form_title is empty but you are using generated_note_form_title, you receive this result:

While you can enter anything into the form_title field, it can create a confusing mismatch if you update the generated_note_form_title but forget about the form_title and someone else reviews the document.

'Analyze Survey' should detect your title in either location.
