Allow Capitals in 'name' column

If you base a survey off of an existing feature layer, the 'name' column can contain capitals, whereas if you create a survey from scratch capitals are not allowed.

This makes processes difficult. Even helping another user troubleshoot their form can be impractical or even impossible depending on the number of fields they have with capitals.
