Embedding Survey with Live Response into StoryMaps?

Hello! I'm looking to embed a questionnaire and an immediate visualization of the responses into my StoryMaps project. I'd love to be able to use an app like Survey123 and have it appear and record responses within the window that immediately appear or even record the responses and email them to the user. Is this possible with Survey123 or is another app attachment better to use for this purpose?

It's been done before. I would suggest reading into live use cases like with embedded surveys through dashboards and similar articles related to storymaps here in the esri community and elsewhere. After the first two or three months of covid this workflow you are describing was wildly popular.
