Multiple Quick Capture Issues

We utilize Quick Capture to conduct windshield damage assessments during storm events. Several users have reported numerous issues today. 

Some are being logged out while using the mobile app, others have reported collecting data, and it shows as sent from their device, but it is not populating on the map. I had a user who submitted 3 records and then collected 10 more, just for those 10 not to show up in the feature service. 

We've been using Quick Capture for several years with no problems, and now all of a sudden, there have been a multitude of issues. 

Has anyone else been experiencing these types of issues? Any thoughts or suggestions
