Smart Editor - Name of Features

I have a question about how to use the Smart Editor in WebApp Builder.  I build a feature layer with my fields, create a map out of it, and then add that to my WebApp.  I then add the Smart Editor, and it has the layer name, a hollow circle (for point data), and "New Feature" under it. 

However, I have seen others use the Smart Editor, and they have a custom icon and the name of the feature beneath it.  I cannot figure out how to do this - I always just have "New Feature" with the hollow circle.  I feel like I'm missing something simple.  Can someone please assist?

@AbigailEbelherr  Make sure you've created feature templates for the layer. You can do this in the web map, follow these steps: Manage feature templates—ArcGIS Online Help | Documentation
