How do I add checkboxes to geoform on ArcGIS Server?

How do I add checkboxes to a geoform we have hosted on our ArcGIS Server?  The fields have a yes/no domain applied but I'm not sure if that is correct or not.  Is there specific code I need to add to the configuration files?  If so which ones?

Yeah, this would be a great option to have.  I emailed some of the esri staff about this.

There is an option to use check boxes instead of dropdowns in the geoform configuration menu. When using the geoform builder, on the "Fields" option (number 5), click on the cog icon next to it.

Then select "Display As" and choose radio button. You can set up your domains to have a yes/no/all coded values.

The only way to access the Radio Button option in the form field's settings is to ensure there's a domain associated with the field.

There is a domain. I can make a menu, but for some reason in this domain, it does not show an option for radio button.

The choices are select menu and filter select …

It must be something with my domain but I can’t figure it out – it was made the same way as the other domains in my gdb…
