Adding Leader Line to Text that already has a Marker Text Background?

I am trying to add leader lines to the default ESRI Highway Symbols (Interstate, US Highway, Etc.).  The highway shield is a Marker Text Background.  How can I add a leader line to these?  I have read the help and it just says to click on your text and pick add leader line.  This does not work.  I would like a Simple Line Callout but that over rides the Marker Text Background.  Can the leader line just be a text blob not associated with an annotation label?  I remember doing this in 9.0.

I am using ArcGIS Desktop 10.0 SP3.

I am having the same problem in ArcMap 10. I have created a custom Marker Text Background and I would like to add a leader line to it. However, it appears to be an either/or. Is there a solution to this?

That's correct. Unfortunately, at 10x, the callout can only be a marker background or a leader line, not both. We are looking at changing this in future versions of Arcmap.
