Geoplanner embedded in portal 10.5 , but i cant find it in portal 10.4.1 !!
where i can find the installation ?

Try logging into the My Esri site with your customer account # and go to software downloads,
https://my.esri.com/welcome
I believe you should be able to download it from there. You will also have to import the license into Portal and assign it to a named user account.
