Photo attachment mix-ups in Collector

I have configured a web map that inspectors use to collect data on a Hosted Feature Service through ArcGIS Collector (Classic), generally in offline mode but not always. There is a point layer with related tables, two of which are the forms that are used to collect data and photos. The app has been working fine up until recently. The inspectors are reporting that photos are getting mixed-up, the wrong photos are appearing in a point's related table. The photos are being added from the photo libraries on their phones, and sometimes they will have the same name, e.g. "Photo4". The photos also come through sometimes as JPEG and others are PNG. 

Has anyone else encountered problems with photo attachments being incorrectly linked? As the team continues to provide more instances of this occurring, it may be easier to drill down to the root cause, or it may just be a bug in ArcGIS Collector (Classic). Any advice is appreciated.

Can we confirm that photos are actually being attached the wrong feature? We are having a maybe similar issue where people using a web map to review attached photos on Power pole points in a web map, see the photos start to appear as being on the wrong point/pole. But it seems more to be a caching issue than the photo is actually attached to the same point/pole. After refreshing the browser or waiting long enough the pole will eventually show up as correct. 

We have also been able to reproduce this issue (however only in Firefox browser at this point) and we have videos from our client with this happening as a good source of proof, happy to share these directly with Esri if requested but cannot share publicly.

Hi All,

A fix has been applied to ArcGIS Online recently to mitigate this behavior. The mismatching of attachments should no longer occur. This scenario could have been encountered when/if the user experiences a sync failure, perhaps due to a network timeout and loss of connectivity during sync, etc. where the local edits w/attachments have been pushed up to the server but no feedback was received by the client indicating the sync was successful. In this case you may attempt to submit a second time by tapping 'Retry' in Collector resulting in the mismatched attachments. 

Hamish Mills‌ You may be encountering something similar but it doesnt sound like the same issue. It would be useful to point this out to Esri Support if possible.
