Collector Attachments not working on IOS

when creating a feature in collector and selecting the attachments (camera) option, i can take a photo and select use photo, but when i get to the option to select "done" it is grayed out. Any ideas, adding attachments worked about 2 hours ago, but now it does not work. i am using a map downloaded for offline use.

Works on my android phone but I still need it to work on the Ipad

Can you still tap on the Done button and have the attachments added to the feature?
