Custom Print With Print Task

My customer wants some thing like given below image as print pdf file.how to make like this by the help of print task.i am using 3.28 Arcgis API version.please help.

Hi Nadir. I think what you would want to do would be to create a custom print template and publish it using ArcMap or ArcGIS Pro.
