add LineOfSight as custom widget into appbuilder 2.16

I am novie to the area. I try to add the 3d lineofsight widget as custom widget into web appbuilder, I tried to use the Demo widget structure, but I failed to make it appear in the widget dialog box as it should. I also don't get the "expand" button, and I don't know how to "skip" the need of the expand button. I'd appreciate to get some code example how to correctly add lineofsight to the web appbuilder? Any help will be highly appreciated.

I attached screenshot showing that the widget is not built ok in my appbuilder.
