Probably the easiest way to do this, would be to create a geoprocessing service and use the Geoprocessing Widget in the WAB... All parameters that you define for the geoprocessing service will appear in the interface:

Geoprocessing widget—Web AppBuilder for ArcGIS | ArcGIS

Hi - I was wondering if someone has developed a widget similar to what I'm looking for, or could give me any advice (I have no experience writing code!). I need to replicate the functionality of a weighted overlay procedure within a Web App environment - the widget would be an interface that allows the user to adjust the weights of a set of rasters comprising the overlay, using a set of sliders from 1-0, where a query returns the weighted sum of adjusted pixel values (the sum of all weights must = 1, so weights of remaining datasets would need to be automatically proportionally adjusted). Has anyone done anything like this? Any assistance would be great!
