Print Attributes to Custom Layout or Report

I've been looking for a widget, or sample pieces, that will allow you to query a feature then print the attributes of that feature in a custom form or report. I find query widgets and report widgets, but not exactly what I'm looking for. I don't want the attribute info to be in tabular format which will come with a downloaded CSV. Can anyone point me in the right direction?
