Citizen Service Request Locks up on Splash Screen

I'm working on configuring the Citizen Service Request template and when I launch it it seems to freeze up on the splash screen. In the background I see the maps and data load, but I can't get rid of the splash screen. I'm using the Citizen Service Request for 10.1, with ArcGIS Server 10.1. Thanks in advance!

Can you check to see if you are using the .net 4.0 application pool? Also this will happen if the layers are failing to load.

Hi Mike,

Thanks for the response. I checked it and I wasn't running 4.0, I was at 2.0. I made the change and it's still doing the same thing. Would this have anything to do with the service request layer? The file I'm using for this app is not from the local government model, but I made the changes in the application for the fields to correspond. If I missed a field somewhere would/could it cause this issue?

It has to do with the Citizen Service request layer. Because I was using a feature class that was not part of the LGIM it was freezing up - obviously somewhere I missed some code.

Has this been identified? I found the same issue with 10.1 and 10.2. Testni in Chrome and the only request that has issues is one to http://services.arcgisonline.com/ArcGIS/rest/info?f=json I open this url in a seperate window and get a response, but app cancels the request. Thanks for any info. (ArcGIS Sever 10.2, using the 10.2 Citizen Service Request template, .NET 4.0)

I am also having issues attempting to customize the Citizen Service Request. So far I am finding it to be unstable. When I make a small change it bombs. But then I also noticed even if I don't make any changes it does not consitantly start up.
Am I missing something, is there a best practice or guide to customizeing the Citizen Service Request map application?

Saving a local copy of dojo and pointing to that instead referring to it on arcgisonline.com appears to have solved the issue for me.
