GeoForm Attributes Missing

Why are attributes not showing up in GeoForm?

They were in the form I disabled 3 other attributes we didnt need and when I previewed it all the previous data was lost from the Fields ( Hint text, Alias changes, order , Etc.. ) and even though I had the fields enabled they still did not show up in the preview.. only change I made to the Form was adding the basemap toggle button and disabling 2 attributes I no longer needed.

I had a similar problem with my form. In my case the issue was that I had default values populated. When I removed the default values from the database the fields showed up in the geoform.

Check your database and see if you have any default values.

Remove the values, create a "new" don't overwrite the service. load the service in the field and see if the missing fields are there.
