topic Examples of custom survey form to pass variables to Geoprocessing service in Developers Questions

We currently have a python script we manually pass parameters to for generation of points in one layer and table entries in another. When a major storm comes through, we have to tell the script if this is a pre or post storm report and provide a name (hurricanes/tropical storms). My DBA doesn't want a a survey123 and doesn't envision the user directly interacting with feature classes. He just wants to pass variables to a GP service.

We are working on creating a geoprocessing service to try and make this process available to non--GIS users. The interface needs to be simple and have those handful of choice lists. Has anyone built anything similar?

Ex. flow: Q1. Pre or post storm assessment?

Q2. Was it a named storm? (follow-up question based on this yes/no)

Q3. If so, what was the name?
