I am wondering if it is possible to create dimensions in arcmap with python, arcmap version 10.7. I have looked and I see functions to list dims but not create them.

Also when I create a dimension in esri dimension feature class the dimensions are gone when I reopen the map I get error "can not initialize COM object" then has layer name for dimension layer.

You have this posted in ArcGIS Pro, but mention ArcMap

For Pro, these are the access points to dimensions

Dimensions in ArcGIS Pro—Dimensions | ArcGIS Desktop

Arcpy can be used to run tools in ArcToolbox, if the process involves wizard, then no.

If a tool in arctoolbox exists, then there will be a code sample at the end of the help topic for the tool.l
