Create Address Locator from shapefile crashes in ArcMap 10.2

I just installed ArcGIS 10.2 for Desktop. When I try to create an address locator in ArcMap or ArcCatalog from a shapefile, it crashes every time. I've done this before in version 9.3 and never had a problem.

Here are screen shots of my Create Address Locator screen with all of the settings that I'm choosing:

[ATTACH=CONFIG]30333[/ATTACH][ATTACH=CONFIG]30334[/ATTACH][ATTACH=CONFIG]30335[/ATTACH]

I've also attached a zip of the shape file I am trying to use.
Am I doing something wrong or is this a bug? Is there a workaround?

Mario,

If you are seeing a crash you should contact customer support. I was able to build a locator using the latest version of ArcGIS (10.2.1) with no issue.

Thanks Brad,
I did contact support yesterday and they are looking into it. Out of curiosity, did you create the locator in 10.2.1 using my shapefile or another shapefile?
