ArcPad Data Manager will not allow template .axf to be chosen

Hello all-
I am using ArcGIS 9.3.1 and ArcPad 8.0.
In the ArcPad Data Manager toolbar when I go to the Configure Project for Geoprocessing and select my layers to export, I am not allowed to select existing forms or scripts using the checkboxes to the right; they are all greyed out and do not allow me to make any changes.

I am selecting the layers to be .AXF and editable, and have created a custom form for these data in ArcStudio that needs to be used in the field.

Also, this was working last week, so I know it can... I am not sure what I seem to have inadvertently changed.

I am using ArcGIS 10 Sp2 and ArcPAD 10 R2 Build 30
Selecting an Layer Definition Template is only working if you use
"Check Out for disconnected editing in ArcPad"

Currently, the only option available which allows you to select your custom forms is Disconnected Editing. The request for custom forms for shapefiles (which used to work in the past) is listed under the following bug:

CQ00312821: Layer Definition Template checkbox is no longer enabled for shapefiles within Get Data For ArcPad wizard
