Is there an out-of-the-box capability to automatically upload the submitted form as an assignment to Workforce for ArcGIS? I was able to find this <A href="http://solutions.arcgis.com/local-government/help/crowdsource-reporter/get-started/create-workforce-assignments/">link</A> on a similar thread and I am working on implementing our workflow this way.
