Create a new record icon is not visible in SMART EDITOR in Web AppBuilder

I have 2 feature classes and 2 related tables

line A (feature) related to table A (table) with one-to-many relationship.
Point B (feature) related to table B (table) with one-to-many relationship.

In Smart Editor:

I am able to add a related record in table B as the icon for adding a related record appears in the SMART EDITOR. I am unable to see the icon when I want to add a record in Table A. The SMART EDITOR window does not show any related records for line A. I have checked the settings of my SMART EDITOR widget and the option to view all related tables and records is checked.

In popup:

The popup for line A sometimes shows 'no related record' in different browsers. However, there is definitely a record in Table A for each feature in Line A. Mostly all the relationships are correctly viewed while opening the app in Incognito mode in Google Chrome or Microsoft Edge. However, this issue is not consistent, so I am unable to find the cause of this issue. Has anyone faced something similar?
