3.5 Custom Header Controller Widget

I have been trying for a while to make a custom header controller widget, but nothing ever works correctly. I found a sample on here for the Go Electric fake site, I really like that. Is there anyone out there that has a custom header controller they would like to share or help me out to make my own? Thanks

What files have you been editing which is not working well for you?
