Survey123 report template: incomplete field parse to Individual template download

Survey created in Survey123 Connect version 3.12

I am having several issues with the report template.

My survey structure is flat - no repeats.

Firstly, when I download the Individual report sample template there are just a few fields parsed to the report - many in the survey are not parsed.

Secondly, I edited that file to include all the fields I want in the report but when I try to select that template document I get the following error message:

'purpose' is a field in my survey (choice list also named purpose). As with previous posts from 2019/2020, if I remove ${purpose} from the template it moves on to error on the next field.

I've included my formatted xlsx, the report sample template and the edited template.

Any help appreciated

You need to close your image with ${/} :
