    <description>&lt;P&gt;Excel files aren't supported it has to be a CSV.&lt;/P&gt;&lt;P&gt;Can you share the CSV files and xlsform? Usually these issues are tied to formatting of the CSV. Otherwise the screenshots look OK.&lt;/P&gt;&lt;P&gt;You also don't have to use underscores in select one. The S123 team have confirmed that it's ok to have some special characters including spaces for single choice. It's only multiple choice that has the no spaces requirement.&lt;/P&gt;</description>
