Crossed image icon, unable to take a picture

Hi community,

I'm having troubles with my survey form. In some cases, a crossed image icon appears instead of the camera icon. The button used to work fine, I don't know exactly what changed since.

Any ideas why?

I just received this screenshot from a field user. In his case, the button works for the first image but not for the following ones. Other users with the same form version do not experience this issue. They are all using the same devices.

That icon appears when a photo question is read-only, could that be the case? Are your users adding new features, or editing existing ones?

Hi Josh, thanks for your reply. The questions are actually not marked as read-only. And the users are both editing and adding new features. The only parameters for the questions are :
body::esri::style method=camera, and bind::esri::parameters fileName=concat(${building_id_calc},"_front") fileNameVisible=true allowRename=false
