Survey123 Unable to create replica "Exporting Domains Failed"

We are trying to pull down survey features along with their 4 attached photos. The export to FGDB does not work so we are using the Create Replica work around other have been using. When we run the create replica process we get the following error: "Exporting Domains failed" I haven't been able to find a lot of info on this... anyone run into the same issue?

I'm going to loop in https://community.esri.com/community/gis/web-gis/arcgisonline‌ as well since this is related to the feature service. Can you provide information on the feature service- the JSON representation of feature layer endpoint(s) would be ideal.

We were missing a name in one of the fields in XLS that we were using images as the buttons. Once we removed the images and updated these fields things worked..
