Double items created in AGOL content when generating Survey123 reports

Any idea why there are duplicate items being created when generating Survey123 reports? I've noticed this several times now. I definitely only clicked to generate the reports once, but there are two items in my content in AGOL:

In the Survey123 website if you open the recent tasks are there two duplicate tasks or just one?

Hi @ZacharySutherby , thanks for the response! There is only one task. When I examined it and clicked more details, I see this information:

I just tested reports on another survey (both a single record, and multiple record task) and I actually didn't get the duplicate items. So maybe it's something unique to this survey...
