Survey 123 Error Code 10500 Database error

I am having an error appear when I am trying the submit/ edit a survey after it is publish the error message is as follows:

I've been look at this for hours and am not completely lost and dont know what to do. all fields show not errors and Ive even made duplicates of the excel sheet to see if there was anything that popped up but to no avail. Any assistance would be greatly appreciated.

Hi @mjennings323 , couple of questions:

What version of Survey123 Connect are you using to publish the survey?
Are you publishing to ArcGIS Online or ArcGIS Enterprise?
Do you have a sample of the XLSForm?

I am using Version 3.17.55 and am trying to publish it to portal

Just based off your screenshot, looks like you might have issues with the field names:

date
block
segment
begin

Just for the sake of troubleshooting, can you please give those fields a more unique name and publish a new survey from the XLSForm, and then test to see if survey submissions are successful?
