Error Generating pdf report

I'm getting the following error when I attempt to generate a report submitted by a user. I can generate my test reports (.docx and .pdf) just fine but this error occurs when I try to generate a report. any ideas on where to start troubleshooting this?

Hello @DMatusze,

Would you be able to provide a copy of your surveys XLSForm and report template for testing on our end? Does this issue occur for all records in your data or just specific records?

Here's a copy of the XLS and the template. Sorry, its quite a long survey. The reporting template works just fine when I submit a survey from my pc and my android device. The issue occurs when a different user submits a form I get the error message I screenshotted in my original post when I try to retreive a PDF or word copy of the report.
