Calculates Stopped Working

Hello,

I have calculates that previously worked, but then randomly stopped working within the last two weeks.

They seem to work for some users but not others (the issue is spreading though)
The calculates work when I copy the old XLSX to a brand-new Survey
Surveys are in Connect and accessed via Field App
- Connect and Field App are most recent versions

I have an open case with Esri, but wondering if others have noticed similar behavior. This is a rather critical issue for us.

This is an example of calculates not working:

The user made a selection which was saved (agree_name), but nothing was found with pull data. This user submitted 10 items. Half didn't work. The other half did.

Edit: Seems like there are two separate issues here. This particular project's lack of data is related to a bug. I have created a separate post regarding this bug.

Here is a comparison of some Meta Data...

The only difference between the submission that worked and the one that didn't work is the "header_utc_date" field. This is a hidden field that the user cannot change. Despite "header_mode" saying that this is a "new" submission, I would guess that the user actually went to the Sent Folder and copied a previous submission for a new survey.
