Preload image for user annotation

I have a Survey123 survey created, but need a way for the user to annotate a standard diagram. So for every survey that is completed, this same diagram needs to pop up and allow them to add markup, then save/submit.

I know how to use annotate to allow for images to be marked up, but it always makes the user upload a picture or take a photo with their camera. I need the diagram to load for the user automatically. I tried using the Note feature and it brings up my diagram image, but cannot be annotated.

Those Sample surveys are awesome! That showed me everything I needed to know after searching online for 3 hours. Thanks!
