Pulling attribute field data for instance name

Hey guys,

Here's my issue:

I have a feature service layer form and I would like to use the label names which is in the feature layer attribute as my instance name. How would I pull the data from the attribtue table to make it as my instance name?

Here's the excel of my survey form that is linked to the feature layer, as you can see i tried pulling the data and made that calculation (last row) but it didn't work.
