Unknown Question type error publishing survey from feature service

I am trying to create a survey from an existing feature service. The service contains a point layer and a related table. I want to only use the related table to collect information in the survey. I'm trying to create the survey in Survey 123 Connect and I receive the following error (see image). I've also attached a view of the spreadsheet highlighting the field that is causing the error. The service publishes with no errors in ArcMap and works without errors in Collector. Any help regarding the error and what I can do to use only a related table in a feature service for a survey is greatly appreciated.

No spaces allowed in list names or field names. Also no commas allowed in the list it self.

Field names did not have spaces but the domain did. Your answer is correct. No spaces in field names or domains allowed.
