Survey123 Report Error

I have a Survey123 report created by a co-worker that has a location map embedded into the report. When I attempt to run the report, I receive the following error.

An error occurred when rendering by the report engine. Failed to print map for ${maplocation}. Error: Failed to load web map: 56b509570aa84893ae8658e4852e5cd1. You are currently signed in as: 'undefined'. You do not have access to this resource: https://services6.arcgis.com/....

I am logged in to Survey123 to access the report, and I also have a tab in Google Chrome open and am able to access the map that it references. I am also able to see the location map within the Survey123 "Report" tab. Any thoughts?

When you open the map in a browser, do you get any error messages like "Cannot load layer: ______"?

Many of my basemaps include feature layers that are publicly shared, but I am not the owner of. If the layer is offline for maintenance or unavailable for any reason, I get an error on report creation as well.
