Form view map glitch

In the Form view of the Data page of my survey, the map has a glitch. When I click on a survey point on the map, a random survey appears on the right side screen panel with info from any of my 20 points. If I click on the same point several times, it sometimes eventually gives me the correct info for that point, but again, it's random.

Map Viewer does not appear to have the same problem.

Any ideas what's going on?

Hi @KD123,

Are you able to provide screenshots or recording of the issue when it occurs?

Also are you able to share your XLSForm (xlsx file and files in media folder) to see if we can reproduce the issue with you survey. Or can you share the feature service/data so that it can be recreated/tested?
