New Survey won't open in Web App (missing .webform file)

I have been away from AGOL for a few months. I have not had this issue before. I downloaded a survey from Esri and published it through Connect.

This is the error I get when I try to open it in the web app:

This survey cannot be accessed in the web app. Please contact the survey owner to republish the survey with 'Create web form' enabled. Error: Missing .webform file.

I checked the files in Connect and the .webform file is there. I do not even know where the "Create web form" checkbox option would be that the prompt is referring to. I do know that it is being created when I publish it since it says it is being created. I am stumped.

Much thanks
