Table not found Survey 123

Hey,

I am trying to publish a survey and I keep getting this error of Table not found.

this survey has been given to me by a company which we are working with. they say everything is fine and should work ok. I have checked it against the feature service and everything seems fine.

Any help would be much appreciated.

You should probably have a look at the Settings tab of your survey's Excel design file. The form_id should match the destination table / layer name.
