Republishing forms and data loss

Hello, I have apparently lost a survey form from our organization level sharing. This form had a significant amount of data associated with it, which I have backed up. I have the exact same form still in "my content" (I was the original creator) and would like to just republish to get it back to the organization. When I have started to republish, I get a warning about how the data may be overwritten, but the form has not been changed in any way. If I republish, will we lose the data? any suggestions for getting the form back out, short of recreating it as a new version?

If the form and associated feature layer appears in your Content (in ArcGIS Online or Portal), it sounds like the sharing on the item may have been changed.

I'd encourage you to check out the sharing settings on your form to see if it's still set to the Organization level.
