Submission URL to View Layer Compatabilty Error

Hello, I have created a survey in connect based on a feature layer (specifically a feature layer view) in Survey123 Connect. I am ready to publish but I keep getting errors that the submission URL is not compatible (a few different errors depending on the attempted url). I have tried using the documentation, but I'm not sure if I'm interpreting it correctly.

For the content root:

If I go to my organizations homepage(https://utah-em.maps.arcgis.com/home/index.html ), is the content root "https://utah-em.maps.arcgis.com/" or is it just a part of that? or is it something else all together?

For the itemID:

If I go to the view layer that I am trying to publish the survey to (https://utah-em.maps.arcgis.com/home/item.html?id=309d8ea304304512847cecebcc2dc8fd ) is the item id "309d8ea304304512847cecebcc2dc8fd"? or is it associated with the URL at the bottom right of the feature page (https://services6.arcgis.com/KaHXE9OkiB9e63uE/arcgis/rest/services/UCIP_Attempt13451223_View/FeatureServer ) with the ID being KaHXE9OkiB9e63uE? Or something else?

What makes most sense to me is https://utah-em.maps.arcgis.com/309d8ea304304512847cecebcc2dc8fd, but that returns an error. When I swap it out for different combinations of these, I get more compatibility errors (though sometimes different).

Here is the little note I always keep for mine. I never use the org part (utah-em).

First part is
https://www.arcgis.com/sharing/rest/content/items/
Then the Feature Service Item ID like
cfb9cc4b7e454e5d9ee044440fd53626

Then use this in the forms
https://www.arcgis.com/sharing/rest/content/items/cfb9cc4b7e454e5d944410fd53626
