TypeError: i.getNodeFromItem(...) is null error in Survey123 web app when using select_one_from_file question type

In version 3.12, I'm using the new select_one_from_file option and receive the following error in the web form, but not in Connect or the field app:

I have stripped everything but the one field from the survey to verify that the select_one_from_file question is the problem. The csv file referenced has been added to the media folder, AGOL as a CSV file, shared, and linked within the survey.

Could you try the survey on survey123beta.arcgis.com next week? We will update the beta website later this week and it should fix your issue.
