    <description>&lt;P&gt;In version 3.12, I'm using the new select_one_from_file option and receive the following error in the web form, but not in Connect or the field app:&lt;/P&gt;&lt;P&gt;&lt;span class="lia-inline-image-display-wrapper lia-image-align-inline" image-alt="WhitneyWeber_0-1618580889981.png" style="width: 400px;"&gt;&lt;img src="https://community.esri.com/t5/image/serverpage/image-id/11090iC753FDE360E4DFAC/image-size/medium?v=v2&amp;amp;px=400" role="button" title="WhitneyWeber_0-1618580889981.png" alt="WhitneyWeber_0-1618580889981.png" /&gt;&lt;/span&gt;&lt;/P&gt;&lt;P&gt;I have stripped everything but the one field from the survey to verify that the select_one_from_file question is the problem. The csv file referenced has been added to the media folder, AGOL as a CSV file, shared, and linked within the survey.&lt;/P&gt;</description>
