Embedding a Survey123 form on a webpage using Survey123 Web App JavaScript API

So following this link to embed a Survey123 form in a web application using a JavaScript API: https://community.esri.com/groups/survey123/blog/2020/03/05/introducing-the-survey123-web-app-javascript-api

However it simply does not work. I'm not sure when I'm doing wrong here I have where the API is linked:

And here is where I am trying to embed the form:

Currently I am simply using an iFrame however I want the form to be of a certain height, but this creates an issue when mobile users needs to sign, they click the signature box and this loads in the iFrame a smaller box which they cannot scroll to. If anybody can help with either the API issue but mainly I'd like to circumvent the signature issue, that would be appreciated.

Did your browser set to block 3rd party cookies? What error did you see on the page or browser console?
