Error 500 - cannot create feature class

Hello,

I am having a hard time trying to publish the attached file. I get the error code 500 (cannot create feature class) and I was wondering what causes this? I have tried to read some of your other posts regarding this, but I cannot resolve the issue and was hoping for a better explanation please. I keep getting this error with other forms, so I am desperate to learn of what I can do to successfully publish these forms. Any help would be greatly appreciated.

Hello Alma,

I looked at the XLS you attached and it looks like "easeofuse" may be the root of our problem. It is used twice as the name for two different select_one questions. After changing one of the names to "easeofuse2" I was able to publish the XLS.
