Null Error when generating report (beta) in Survey123

I have a feature layer (>2,000 points) with several related tables used to conduct inspections via Survey123. I've set up a printing template (devised from the sample template), which pulls data from the related tables and produces a report.

The current workflow is:

User navigates to https://survey123.arcgis.com/surveys > data > generate report (beta) > selects a point in the map view > selects a template > generate

This workflow has worked in the past. We've tried selecting multiple points, changing the template and uploading a new template with no luck.

Here's the error we're seeing: "An error occurred while generating report(s). [null]

If anyone has any feedback, I'd greatly appreciate it!

Could you share the XLS file and DOCX template so that we can attempt to reproduce?
