Pulldata and ODK error

I am trying to use a csv file in the media folder to pull into a question to auto fill. I am unable to share the exact data file due to the nature of our work however I have included generic screen shots.

Main problems:

pulldata calculation error
survey unable to locate csv
is the csv file correctly formatted
choices for the name list matching the csv

is this how it should be saved into the media file?

Hi Janelle,

Apologies for the delay. The file used in pulldata should be a CSV file placed in the media folder. I would encourage you to check that it is a CSV file by opening it outside of MS Excel; a text editor such as Notepad should be able to view the file content. Also, I would make sure there are not any commas in the data values; that can throw off the positioning of the data in the CSV file.
