It just sends me to the main survey123 page.&lt;/P&gt;&lt;P&gt;&lt;/P&gt;&lt;P&gt;Be sure you do not have any 'illegal' characters for your form or app. Earlier someone posted that they used an ampersand (&amp;amp;) and that gave them the same result with the blank collection screen (like you're seeing).       <title>Error using Survey123 for ArcGIS app</title>

I had one titled "Hydrant_123" and it showed up blank. I tried "Hydrant-123", "Hydrant123", and even "Hydrant", and still got blank surveys. I have another named "Valve-123", and it works just fine.&lt;/P&gt;&lt;/BODY&gt;&lt;/HTML&gt;</description>

I made an entirely new survey and made sure every single thing on it was "legal"... and it worked. Thanks for giving me that idea!&lt;/P&gt;&lt;/BODY&gt;&lt;/HTML&gt;</description>
