Hi - It looks like the form_id is pointing to a layer called: NTMWDDiversionTab. However, this does not exist in the feature service that is being referenced in the submission_url. This is causing the error.

I know I have to replace it with another service, it was set point to table by a relationship. Since it was acting up I change it point to point with a relationship.
