Publishing error code 400 - Unable to create folder

Hello. I am attempting to publish a survey but I am getting error code 400 - Unable to create folder. All values in the name field are unique, and I am not using any reserved keywords. I've tried deleting the feature service and the .iteminfo file and republishing, but this didn't work. I've also tried publishing a copy of the survey, but I receive the same error code. There must be a problem with the xls, but I've looked it over multiple times and can't locate any potential issues. The xls is attached. Please help. Thanks.

Hi Chris,

The most likely cause of this is an initial publishing failure, which creates the folder in your ArcGIS Online/Enterprise account. Delete the folder in that account and attempt to republish.

After deleting the survey from the Connect app and ArcGIS Online, I was able to successfully publish the survey. Thanks for the help.
