topic How can I format a question that needs multiple fill-in-the-blank answers? in ArcGIS Survey123 Questions

I am currently working on simplifying PS&E forms (used to authorize plans for construction) for a client by using a Survey123 Connect form to autofill data for them to view when they go into the field. I have figured out the pre-populating by using pulldata() for the questions on their survey that we have data for. I am now struggling with the design for some of these questions that are in a table-like format and require multiple fill-in-the-blank answers. The screenshot below shows an example in their current form.

What is the best way to format this?
